Dear KPH Community,
Humboldt Park Health is now offering the Pfizer COVID-19 vaccine for anyone 16+. To get your vaccine:
- Walk into Humboldt Park Health at 1044 N Francisco Ave – 1st Floor
- Monday through Friday between 9AM – 4PM or Saturday between 9AM- 2PM
Spanish interpreters will be available on site and language line will be available for all other languages. No social security security number is needed. Bring an insurance card if you have one, but vaccines will also be provided for free without one.
